MBayKids Teacher of the Month Nomination Form

Does your child have a teacher who is making a real difference in his/her life? Help your child nominate his/her special teacher special instructor for the MBayKids Teacher of the Month Award. Both your teacher, your child and your child’s class could win some really cool prizes – and your nominated teacher and child could be featured in an upcoming issue of Scoop! magazine.

Nominations may be made for any teacher in grades K-12 who teaches at a public or private school in Monterey, Santa Cruz or San Benito counties.

To nominate a teacher, simply complete and submit the online form, below, OR download and print a copy of this form and mail to: MBayKids Teacher of the Month, 1172 S. Main St. #326, Salinas, CA 93901 or fax a copy to (888) 751-2724. All submissions become the property of MBayKids. Parents of the children whose nomination has been selected for further consideration will be contacted by phone and/or by e-mail.
